Initiating Services
Please call 616.510.1267 to initiate services. A pediatric therapist will come to your home to complete an initial evaluation with your child and discuss further treatment.
A script including diagnosis, physician signature, date, and treatment is required prior to evaluation. Scripts may be faxed directly to Kid at Heart Therapy at 616.399.4387. If needed, we will be happy to assist you in requesting a script from your child’s doctor.

We are established as a participating provider with most major insurance companies. Our billing department works closely with insurance companies to determine eligibility and coverage, and will bill your insurance company directly for our services. Not all therapy services are a covered benefit in all contracts. We will gladly discuss your insurance benefits and answer any questions you may have as to the involvement of your insurance in receiving this care. If there is no insurance coverage, we will work with you to arrange payment options.
